Use of medical home and medical teams, including promotoras, community health workers, that showcase insurance options and demonstrate welcoming and encouraging health care environment that would empower Latinos through health education and support and opportunities for healthy behaviors/choices. These supports include insurance plans that provide transportation, translation, nutrition, physical therapy, dental and counseling services.
